How do you perform endoscopic vacuum therapy for colorectal anastomotic leakage?
Colorectal anastomotic leakage complicates from 3 to 19 % of colorectal surgery and its management may be challenging! 👉 Endoscopic Vacuum Therapy (EVT) has emerged as a promising treatment modality although results are inconsistent when comparing to a standard management (antibiotics +drainage + eventual diverting stoma).
📊 Lou and Colleagues tried to clarify these outcomes by doing a systematic review and meta-analysis comparing EVT and the traditional approaches.
And this is what comes out:
- Better rate of resolution of anastomotic leakage (RR 1.18 95%CI 1.03 to 1.35; P =0.02)
- Shorter treatment duration (mean difference −30.6 days, 95%CI −43.5 to−17.7;P<0.001)
- Comparable rate of bowel continuity restoration (RR 1.22, 95%CI 0.80 to 1.86; P = 0.34)
- Comparable hospital stay (mean difference −1.7 days, 95%CI −4.2 to 7.7; P = 0.57)
- Comparable complications (RR 1.17, 95%CI 0.52 to 2.80; I2 = 13%; P = 0.71)
💡 Although these finding are promising, the heterogeneity is high among studies and prospective studies are still lacking!
