Endoscopic ultrasound-guided gallbladder vs bile duct drainage for first-line therapy of malignant biliary obstruction Mangiavillano et al.

This international multicenter study compares EUS-guided gallbladder drainage (EUS-GBD) with EUS-guided choledochoduodenostomy (EUS-CDS) as primary drainage strategies for distal malignant biliary obstruction.

📍 Study design & population:

• 🌍 International, multicenter retrospective study

• 👥 291 patients with distal malignant biliary obstruction

• 🧠 Primary drainage performed using EUS-guided techniques • Comparison: EUS-GBD vs. EUS-CDS

(via lumen-apposing metal stents)

🔍 Main findings:

• ✅ Technical success: – EUS-GBD ~96% – EUS-CDS ~99%

• 📉 Clinical success: comparable between groups (~86–92%)

• ⚠️ Adverse events: similar overall rates

• 🔁 Reintervention rates: no significant difference

💡 Takeaway: both EUS-GBD and EUS-CDS appear effective and safe as first-line EUS-guided drainage options in distal MBO, offering flexibility when standard ERCP is difficult or fails.
